Online Vaccination Consent

Dear Gr. 6 Parents,

Fraser Health will be coming to administer our Gr. 6 vaccinations on November 5th. Please use this form to give consent. It saves you having to worry about paper copies. The school nurse has asked that you complete a consent form even if you do not give consent. it gives the option of not giving consent. The reason for this is that if there is no consent form completed the school nurse is required to call parents. However, if you have completed the form ahead of time and indicated that you do not give consent, the nurse will not need to call you.
